Your profile is yours. Here is how that stays true.

Whether you claimed it yourself, an agency claimed it for you, or someone claimed it who shouldn't have.

Who owns a profile

The provider whose NPI it is. Ownership sits on an account with your email address. An agency or your practice staff can hold a manager seat on it, which you granted with one click and can remove at any time. A seat lets them edit and pay for plans; it never lets them remove you, change your email or transfer the profile.

If an agency claimed it for you

  1. The approval email you clicked created an account on that email address. Sign in with it; if you never set a password, use "Lost your password?" on the login page.
  2. On your account page, "Who else can edit your profile" lists every seat. Remove any you no longer want; the agency is told, and nothing they wrote is deleted.
  3. Want the identity check too? Open the claim page for your profile: it is available to the owner at any time.

If someone else claimed it, or an agency claimed it and you want it back

Tell us. Use the form below with the email address you used before (the one the approval or identity check went to), the NPI or your name, and what happened. We check it against our records — every identity check keeps the verified name and the account address — and move the profile to you, usually the same business day. The agency or previous holder is told and loses access; nothing they wrote is deleted.

Prefer to settle it yourself right now? You can also open your record on the claim page and add &reclaim=1 to the "confirmed" link, which runs the $2 identity check; if the verified name matches the record, ownership moves automatically. Most people find the form easier.

If you manage profiles for providers

Ask for access from your account page: find the provider, enter the address they read, add a line about who you are. The email says who asked, what Guide.md is, and that the provider can take the profile back at any time. A provider who declines cannot be asked again by the same account for 90 days. Seats are visible to the provider and to Guide.md; holding a profile a provider wants back is not possible, by design.

Questions

Can an agency keep my profile if I stop working with them?

No. Remove the seat from your account page, or reclaim the profile with an identity check. Either ends their access immediately.

I approved by mistake.

Remove the seat from your account page. If you cannot sign in, use "Lost your password?" with the email the approval went to.

I never clicked anything and my profile is claimed.

Reclaim it: the identity check with a matching name moves ownership to you. Write to us if the name on the record is not yours (a name change, for example) and we will handle it by hand.
